Position Summary: The Nurse Practitioner is a highly skilled professional member of the Interdisciplinary Team with the primary responsibility of providing palliative care consultations and end of life care to hospice patients and their families within their scope of practice. The HCIB Nurse Practitioner has a key role to assist in determining patient eligibility for hospice services. This position has responsibilities with providing and instructing the clinical team in symptom management for end of life patients.

Essential Job Functions

* Participate in the medical evaluation of patients admitted at the general impatient level of care who are residing in the hospital, home and SNF.
* Evaluate and document medical necessity for specific level of care. If patient doesn’t qualify for specific level care, facilitate transfer to appropriate level of care.
* Offer advice and information to staff and referring physicians on medical interventions consistent with the hospice philosophy and plan of care.
* Consult with attending physicians, hospice staff and referral sources as requested, so pain and symptom management for patients is effectively and expeditiously accomplished.
* Provide follow-up evaluation to patients and families and clinical consultation to hospice staff as needed.
* Participate in the review and development of practice protocols proposing interventions that are consistent with hospice literature and research.
* Confirm patient eligibility for hospice services in accordance with hospice regulations.
* Responsible for making contact with assigned patient and/or family within twenty-four hours to arrange a meeting to provide information to the prospective patient and/or family and/or referring professional. An assessment of the patient’s eligibility for hospice services with be completed and communicated to the patient and family.
* Based on patient need, consultation and evaluation of the patient will be completed.
* Responsible for informing the primary physicians and referral sources the outcome of the Nurse Practitioner consultations and evaluations.
* Responsible for documenting interactions between hospice staff, referral sources, patients and families.
* Maintain regular contact with referral sources as assigned.
* Complete, maintain and submit accurate and relevant clinical notes regarding patient’s condition and care. Record pain and symptom management changes and/or outcomes as appropriate.
* Point of Service documentation is an expectation of all clinical personnel for quality care, accuracy, and communication standards. The clinician is expected to appropriately document at Point of Service, have timely synchronization of patient data & effectively communicate with the Patient Care Team.
* Present education and information to members of the IDT and/or medical community as needed.
* Participate in the resolution of interpersonal conflict and issues of clinical and ethical concern as needed.
* Participate in research activities or special projects as assigned and needed.
* Face-to-Face visits as assigned
* Medication management for patient’s enrolled in hospice consisting of:
o Review of Medications, coverage and & collaborative/liaison to pharmacy.
o Review medication appropriateness /alternatives as appropriate, for both new admissions and current patients, on a weekly basis
* Participate in program development with hospital projects

* Minimum of two (2) year's of professional experience in health care.
* Experience in a hospice care preferred.
* Experience in acute care in an institutional setting preferred.
* Experience in homecare preferred.
* Understands hospice philosophy, and issues of death/dying.

Education and Training:

* A Master’s Degree in Nursing is required.

License, Certification & Registration:

* Possesses and maintains current CPR Certification.
* Current Massachusetts licensure: Nurse Practitioner
* Valid driver’s license
* Hospice and Palliative Care Nurse Certification preferred

Other Requirements:

* Demonstrates good verbal and written communication, and organization skills.
* Strong observation skills and problem solving skills.
* Experience working with an Interdisciplinary Team preferred.
