Family Support Service Navigator
Full Time, 37.5 hours per week, hourly

Service Navigator Responsibilities

The goal of this role is to provide support, information and resources and navigation of the systems of care to families and caregivers of individuals with disabilities. With this in mind you will:

  • Administer UCP’s Representative Payee program including Charges for Care and Funds Management.
  • Manage, process, and oversee all aspects of DDS’s Financial Assistance and Flexible Funding program.
  • Administer the Agency with Choice Program in compliance with DDS regulations and guidelines. This includes creation of budgets, attending necessary ISP meetings, HCSIS documentation, supervision of staff, and regular check ins with the family.
  • Support and empower families to develop leadership skills and offer timely and relevant advice and information to all regarding available resources.

Service Navigator Qualifications

  • Bachelor’s Degree in related Human Services field
  • 2+ years’ experience in human service-related employment; experience providing program services in a human services setting preferred
  • Ability to maintain an Adult/Pediatric CPR and First Aid certification.
  • Ability to work flexible hours as needed
  • Negative Mantoux TB test or a clear chest x-ray.
  • Valid driver’s license, safe driving record, availability of private vehicle, up-to-date car insurance
  • Satisfactory CORI, SORI, DPPC, National Fingerprint background checks.
  • Must be willing and able to perform essential job functions through in-person, face-to-face contact as required, in accordance with health and safety guidelines.
