The Registered Nurse (RN) will be an integral part of the Adult Family Care (AFC) and Family Support Multidisciplinary Teams for UCP of Western Massachusetts. This position will work in collaboration with the Clinical Nurse Supervisor and Director of Pioneer Valley Services to ensure the healthcare needs of the members in the programs are adequately supported. This position will work with agency participants, family members, and the multidisciplinary professional teams to ensure plans of care are developed and followed, provide ongoing support and maintain paperwork for all members enrolled.
