The Nurse Care Manager (Outpatient) is accountable for the development, implementation, operation, evaluation and enhancement of the pediatric care coordination programs focused on the provision of care management within the outpatient, pediatric, and primary care practice settings, working collaboratively with the community and care coordination teams, including public schools.

  • POSITION QUALIFICATIONS (Minimum qualifications are required unless stated otherwise.)
    • Experience:
      • Five years of clinical experience in a directly related setting (outpatient clinical or pediatric setting}
      • Three years of case management, school nurse or care coordination preferred.
      • Two years of experience in a supervisory/management or lead role.
      • Previous direct experience in managing or leading government/ACO programs preferred.
      • Previous experience with behavioral health and substance use disorder preferred.
    • Education and Training:
      • Bachelor’s Degree in Nursing, Social Work, and/or related healthcare discipline required.
      • Master’s Degree in nursing is preferred.
      • Understanding of patient-centered care principles, population health management, stages of change, self-determination .
    • License, Certification & Registration:
      • RN License in MA.
      • Certification in Care Management is preferred.
    • Other Requirements:
      • Excellent interpersonal skills and abilities in relating to people at all levels to represent the Health System professionally and negotiate, sell, motivate and coach successfully.
