The Nurse Care Manager (Outpatient) is accountable for the development, implementation, operation, evaluation and enhancement of the pediatric care coordination programs focused on the provision of care management within the outpatient, pediatric, and primary care practice settings, working collaboratively with the community and care coordination teams, including public schools.

Community Care Coordination is an exciting community impact model that emphasizes a team approach to optimize children and families’ care coordination with education, healthcare, behavioral health, cultural and community services to support healthy human development and resilience. With the family at the center and relationships based on respectful listening, care coordination breaks down silos between systems and creates conditions for hope. Ultimately, all families have what they need to thrive; resources are adequate, families can and do access needed services, and community members work to create a sense of belonging and equity. Where gaps in services exist the Community Care Coordination team connects and advocates for additional resources. Working in a team is a fun and hopeful way you can impact your community.
