RN - Preference: Required
The RN Case Manager is responsible for providing case management services for the substance use patients treated with Buprenorphine within a practice providing Buprenorphine treatment and participating in the Blueprint Spoke program.
Requirements: Graduate of an Accredited School of Nursing. Bachelors Degree in Nursing or related field highly preferred. Five (5) years of clinical nursing experience required. Experience in mental health settings or substance treatment program highly preferred. Prior experience in a physician's practice or case management highly preferred. Valid Vermont State Nursing License required. Case Management certifications preferred. Valid Driver's License and Auto Insurance required.
