Description:

The Intake Coordinator has a wide range of responsibilities related to accurate and timely processing of the patient care referral. The purpose of the position is to ensure that all information in the referral packet is complete and accurate before the first visit is made and to facilitate the data entry process of referral information.

Schedule:

  • 40 hrs per week
  • 8:00am-4:30pm, occasional need to work 8:30am-5:00pm
  • Monday-Friday
Requirements:

POSITION QUALIFICATIONS (Minimum qualifications are required unless stated otherwise.)

    • Experience:
      • Two years of experience in a medical office or similar setting required preferably in a home health agency.
      • One year of data entry experience in an electronic medical record (EMR)or use of EMR required.
    • Education:
      • Graduate of an accredited LPN program
    • Licensure & Certifications:
      • LPN - Licensed Practical Nurse
      • DRIVER - Drivers License
  • SKILLS & ABILITIES
    • Working knowledge of health insurance coverage requirements.
    • General computer skills including use of Word, Excel and Outlook required.
    • Occasional travel to a hospital, nursing home or physician office required.
    • Demonstrated evidence of dependability in maintaining a work schedule.
    • Ability to manage time productively.
    • Demonstrated ability to work cooperatively with a variety of people within and outside of the agency.
    • Ability to concentrate and work productively in spite of interruptions.
    • Ability to work independently, problem-solve and follow-through with solutions.
