Description:

Care Manager - Adult Foster Care (AFC)Job Type: Full-time (37.5 hours)
Job Location: North Adams, MA office with caseloads throughout Berkshire County.

$20-$24/hour per hour 

The Adult Foster Care (AFC) Manager is a professional support position responsible for planning, coordinating, and monitoring AFC members.  The care manager’s responsibilities include providing psychosocial and counseling assistance for the AFC program and timely reporting. 

Some duties include:

  • Conducting initial and ongoing assessments of the AFC qualified setting.
  • Obtaining a social history and conducting a psychosocial assessment and evaluation for each AFC member.
  • Select, train, evaluate and supervise AFC caregivers in conjunction with RN or LPN.
  • Participate in the development, implementation, and ongoing review of the AFC plan of care for each member.
  • Conduct on-site visits and associated progress notes with each member at the qualified setting bi-monthly (alternating with the bi-monthly visit by the registered nurse) for Level 1, and monthly for Level II.
  • Review monthly the AFC Caregiver logs.
  • Make referrals to appropriate service providers if a member requires health or social services other than AFC.
Requirements:

Care Manager Qualifications:

  • Bachelor’s degree and two years of clinical experience in the care of elderly or disabled persons, candidates with significant clinical experience to be considered with or without degree.
  • Valid driver’s license, availability of private vehicle, up-to-date car insurance
  • Ability to work flexible hours in order to accommodate the needs of members
  • CORI, SORI, DPPC and National Fingerprint Check
  • A negative Mantoux TB test or a clear chest x-ray
