Description:

Make a meaningful difference in the lives of older adults and individuals living with disabilities. As a Case Manager, you will play a vital role in helping people maintain independence, access essential services, and receive compassionate, coordinated care.

As a key member of our Primary Care and Interdisciplinary Teams, you will provide comprehensive case management services to individuals enrolled in MassHealth Senior Care Options (SCO) and One Care programs. You’ll support adults age 65+ as well as younger individuals with disabilities, working alongside them to understand their goals, respect their choices, and connect them to the resources they need to thrive.

What You’ll Do

  • Build trusting relationships with clients and families, grounded in respect and person-centered care
  • Conduct initial and ongoing assessments to understand strengths, needs, and goals
  • Develop and coordinate individualized service plans that promote independence and well-being
  • Connect individuals to community resources, healthcare services, and supports
  • Collaborate closely with interdisciplinary teams, providers, and community partners
  • Conduct home visits and facilitate care planning conversations
  • Maintain consistent communication and complete timely, accurate documentation

Requirements:

Qualifications

  • Bachelor’s degree preferred
  • Minimum of two years of professional experience working with individuals age 65+
  • At least one year of experience in a healthcare setting serving older adults (e.g., hospital, nursing facility, community health agency, or interdisciplinary program within an ASAP)
  • Ability to work remotely as needed, with reliable internet, phone access, and transportation
