Responsibilities:

  • Under the direction and supervision of the Director of Advocacy & Family Support per the AFC Regulations the licensed practical nurses may:
  • Complete and coordinate all applicable clinical assessments and clinical evaluations;
  • Develop and review on an ongoing basis each member’s AFC plan of care;
  • Provide oversight to AFC caregivers in conjunction with the care manager;
  • Review the PCP Summary Form;
  • Monitor the health status of all members to ensure that all needed AFC is properly delivered;
  • Report changes in the health status of any member to the member’s PCP;
  • Conduct on-site visits with each member at the qualified setting in accordance with 130 CMR 408.415(B)(6);
Requirements:
  • Licensed Practical Nurse with four years’ Two years’ experience in with ID/DD populations or the elderly. Current license in the State of Massachusetts.
  • Valid driver’s license and insured vehicle.
