Position OverviewThe SCO Case Assistant supports older adults and individuals with disabilities by helping them access and maintain MassHealth, housing assistance, and other public benefits. Working closely with Geriatric Services & Support Coordinators (GSSCs) and the Primary Care Team, this role assists with applications, renewals, documentation, follow-up, and administrative support to help consumers remain safely and independently in the community.Key ResponsibilitiesBenefits & Resource Assistance - Assist with MassHealth applications, renewals, and eligibility reviews.
- Support applications for public benefits, housing programs, and community resources.
- Gather, organize, and submit required documentation.
- Track application status, deadlines, and follow-up needs.
- Communicate with consumers, families, public agencies, and healthcare providers.
Housing Support - Assist consumers with affordable and subsidized housing applications.
- Obtain and organize required housing documentation.
- Follow up with housing authorities and providers regarding application status.
Administrative Support - Maintain accurate consumer records and documentation.
- Enter information into agency databases and tracking systems.
- Scan and upload documents to electronic consumer files.
- Ensure compliance with confidentiality and program requirements.
- Perform general administrative duties and other assigned tasks.
QualificationsRequired - High School Diploma or equivalent.
- Valid Driver's License.
- One (1) year of experience in human services, healthcare, social services, housing, aging services, disability services, or a related field.
- Interest in working with older adults and individuals with disabilities.
Preferred - Knowledge of MassHealth, housing resources, or public benefits.
- Experience supporting older adults or individuals with disabilities.
Skills & Abilities - Strong organization and attention to detail.
- Excellent communication and interpersonal skills.
- Ability to manage multiple tasks and deadlines.
- Problem-solving and resource coordination skills.
- Proficiency with Microsoft Office and database systems.
- Ability to work independently and collaboratively.
Work Environment - Primarily office-based with regular computer and phone use.
- Travel throughout Berkshire County required.
- Must have reliable transportation, valid driver's license, vehicle registration, and automobile insurance.
- May visit consumers' homes, healthcare facilities, and community agencies.
