Description: | Community Living Options Coordinator Location: Pittsfield, MA, 01201 Position Overview: The Community Living Options Coordinator plays a vital role in supporting individuals transitioning from nursing homes to community living. Under the supervision of the Assistant Director of Community Living Options, this position is responsible for creating and finalizing Transitional Assistance Plans and Housing Search Entity Plans. The Coordinator provides direct support to participants and engages with the community to facilitate successful transitions. Key Responsibilities: - Develop and finalize Transitional Assistance Plans and Housing Search Entity Plans in collaboration with participants and under the guidance of the Assistant Director.
- Assist participants in transitioning from nursing homes to community living environments.
- Provide direct support and engage with community resources to ensure smooth transitions.
- Complete administrative tasks related to the Community Living Options department and support overall agency operations as needed.
- Support UCP staff by managing tasks that allow them to focus on their core responsibilities effectively.
- Represent the agency positively to members, staff, and visitors through professional and compassionate interactions.
