Description: | The Town of Adams is seeking a qualified individual to fill a full-time Outreach Coordinator position with the Council on Aging. This is a 35hr/week union position with benefits. Under the direction of the Council on Aging Director, this compassionate and community-oriented individual will provide assessment, information and referral, advocacy, case management, supportive consultation, and community resource coordination to older adults, caregivers, and individuals with disabilities. The person in this position assists residents in accessing programs, benefits, and services that promote independence, dignity, health, safety, and quality of life. Working collaboratively with the Council on Aging staff and volunteers, the Outreach Coordinator serves as a trusted resource for residents and families navigating social, emotional, financial, housing, healthcare, transportation, caregiving, and aging-related challenges. Work includes assessing individual needs and referring older adults to the appropriate resources for assistance; preparing reports and correspondence; responding to inquiries, requests and complaints; maintaining and updating files and records; providing follow-up as needed; assisting in program development, volunteer coordination and case management; and performing a variety of administrative and support responsibilities. Successful performance will be enhanced by a candidate who has extensive knowledge of older adults, computer programs and general exposure to public relations as well as a sense of humor and a positive attitude. The position requires a Bachelor’s degree in Social Work, Human Services, Gerontology, Psychology, Sociology, or related field; or any equivalent combination of education and professional experience; a minimum of three (3) years of professional experience in aging services, social services, healthcare, behavioral health, case management, or a related field. Computer literacy is required. - Regularly required to sit for long periods of time, walk, stand, sit, grasp, pull, reach, stoop, talk, and hear; operate a computer, printer, telephone, copier and other standard office equipment requiring eye-hand coordination and finger dexterity; move about department to perform various duties.
- Ability to setup and breakdown event rooms including moving tables and chairs, decorating, lifting objects such as books, audio/visual and office equipment; and computer paper; assist in moving or lifting items such as persons in a wheelchair, walkers and/or other objects up to 50 lbs.
- Possess adequate hearing and language abilities to understand normal conversation over the telephone despite background noise; comprehend and understand conversation with individuals with an accent.
- Possess sufficient corrected vision, under conditions of glare and reduced lighting, to view computer screens, work with details and operate keyboard for extended periods of time to input information.
- Must be able to communicate written and verbally; read documents for general understanding and analysis; with vision and hearing at or correctable to normal ranges.
Minimum Qualifications: - Education and Experience:
- Bachelor's degree in Social Work, Human Services, Gerontology, Psychology, Sociology, or related field; or any equivalent combination of education and professional experience.
- Three (3) years of professional experience in aging services, social services, healthcare, behavioral health, case management, or a related field.
- Other Requirements:
- Valid Massachusetts Class D Driver's License
- Willingness to obtain CPR/First Aid certification
- A clear Massachusetts C.O.R.I. is required in accordance with Massachusetts General Laws
- Regular and punctual attendance Monday – Friday
- Knowledge, Ability and Skill
- Knowledge of care network and of senior care issues, the aging process and related older adult medical and mental health issues.
- Knowledge and understanding of State and Federal programs and other community resources to provide effective and appropriate referrals for older adults and their families.
- Ability to assess and make decisions regarding the welfare and safety of clients and their families and caregivers.
- Ability to communicate in a respectful, courteous, and professional manner.
- Must have good written and verbal communication skills as well as the ability to be flexible and exhibit honesty and integrity.
- Must have basic computer skills- operate email, internet searches, Microsoft Office products, etc.
- Must possess excellent customer service skills with sincere interest in working with older adults and persons with disabilities and maintain strict confidentiality.
- Strong organizational and planning skills.
