This position is responsible for the Case Management of all group employment referrals and members. Responsible for ensuring that individuals served in the department obtain and maintain meaningful employment within the community. Perform all case management tasks including but not limited to completing progress summaries, assessments and goals, attending ISP meetings, completing job evaluations and creating career plans.

Associate degree with a minimum of 1 year experience in a supported employment program or High School diploma/GED with 4 years’ experience working with developmentally disabled adults in a supported employment program or in a position of case management. Ability to complete all initial and ongoing background checks including but not limited to: CORI, SORI, Fingerprinting, LEIE, driving record, E-verify, Physical, PPD and licensing and/or certification checks (i.e., nursing). Valid US driver’s license and ready access to a vehicle. TRAINING REQUIREMENTS: Must maintain all certification needed in the program including CPR, First Aid, ), and BFAIR Human Rights Training. May need to attend workshops, seminars or BFAIR functions as needed for job development. |
