Schedule: 32 hours/week

Location: Hybrid role, located at Neighborhood Health Center and/or Berkshire Pediatrics in Pittsfield, MA when on-site.

The Integrated Behavioral Health Clinician will provide short-term therapy to patients within the context of a primary care setting, addressing the patient’s mental health and wellbeing in a holistic framework. This person will work with both pediatric patients (mostly adolescents) and adult patients, collaborating with primary care providers and staff to provide goal-oriented, patient-centered care. This person will support CHP’s mission, vision, and values and will adhere to compliance protocols as well as CHP’s policies and procedures.

Essential Duties and Responsibilities:

  • Collaborate with all members of the healthcare team in development of the Plan of Treatment, including discharge plans, based on the psychosocial assessment and accepted social work interventions.
  • Develop and maintain thorough knowledge of referral resources throughout the service area.
  • Adhere to all third party payer requirements.
  • Maintain accurate and timely records, including clinical progress notes, according to facility policy.
  • Provide positive customer relations in dealing with patients, families, primary care providers and other members of the healthcare team, referral sources, services agencies and others.
  • Stay current on relevant therapeutic interventions, research, and trends in the field through use of Continuing Education time.
  • Adhere to all department/facility policies, including attending clinical supervisions and case consultations.

  • Education: Master’s degree in Social Work
