The Outreach Counselor provides a variety of supports and assistance to individuals with developmental disabilities in the community and to their family members. They provide guidance and training in developing skills needed to live independently. Outreach Counselors ensure that people supported reside in a safe and healthy environment with proper access to medical and social supports.
Min of 18 years of age. High School Diploma/GED or one year experience in a related field, must possess basic math (addition/subtraction) skills, ability to read & write, communicate effectively both verbally and written, ability to multi-task and exercise good judgement. Ability to complete all initial and ongoing background checks including but not limited to: CORI, SORI, Fingerprinting, LEIE, Driving record, E-verify, Physical, PPD and licensing and/or certification checks (i.e. nursing). Valid US driver’s license and ready access to a vehicle.
