Supported Living Care Navigator
Job Type: Full Time
Location: Pittsfield, MA
Schedule: 37.5 hours week


Position Description:

The Supported Living Care Navigator is a professional support position responsible for planning, coordinating and monitoring members involved in the Supported Living and ABI/MFP Community Waiver Program through Mass Ability (MBY). The Care Navigator’s responsibilities include providing case management to the Supported Living member, including oversight of the PCA program and community integration. The Care Navigator will assume primary responsibility for the recruitment, supervision, scheduling and evaluation of all assigned staff.

Specific Job Duties:

  • Collaborate with Mass Ability (MBY) in providing services to members living independently in the community.
  • Conduct all work in compliance with the rules and regulations stated in the Provider Manual for the MBY ABI/MFP Community Waiver and the MBY Supported Living In-Home Supports Manuals.
  • Supervise the Adult Companions and In Home Support staff employed by UCP and provide oversight of personal care attendants (PCA) employed by members.
  • Oversight of PCA program to ensure respective tasks are fulfilled by Personal Care Management (PCM) agency and Consumer Employer of PCA.
  • Provide surrogacy if requested when a consumer cannot independently manage their PCA program.
  • Recruit and hire, supervise and participate in the performance evaluation of Adult Companions and In Home Support staff.  
  • Ensure that all Adult Companion and In Home Support staff complete orientation on a timely basis and all mandated training/retraining is completed in a timely manner.
  • Monitor and report weekly to the Assistant Director of Community Living Options about high profile/hot button case-related issues from the Supported Living Program as needed.
  • Participate in the development, implementation, and ongoing review of the Supported Living Plan of Care for each member in Supported Living.
  • Attend and participate in all team meetings toward the development and evaluation of an Individual Support Plan for each member in the ABI/MFP Waiver program.
  • Maintain all records for the program and complete care management progress notes as required.
  • Participate in the discharge planning and implementation of services for each member.
  • Make referrals to appropriate service providers if a member requires health or social services other than Supported Living.
  • Provide timely assistance and respond to urgent or emergency needs of the member.
  • Develop an emergency backup and personal care contingency plan for each member.
  • Participation in a rotating on-call schedule for the department emergency cell phone.
  • Provide any service navigation as required for Supported Living.

Qualifications: 

  • Bachelor’s degree and at least two years of relevant experience working in the community with elders, disabled adults or other vulnerable populations. 
  • Ability to work flexible hours
  • Analyzing data, effective listening, facilitating meetings, monitoring activities, planning and problem solving and a working knowledge of computers and basic computer programs.
  • Knowledge of age-appropriate activities and behaviors, technical writing, defining issues and drawing conclusions, treatment modalities and assessments.
  • Knowledge of community social services, education and health service agencies
  • Valid driver’s license, availability of private vehicle, up-to-date car insurance
  • Ability to work flexible hours
  • Participation in a rotating on-call schedule for the department's emergency on-call cell phone. 
  • Proficiency with Microsoft Office/Office 365 and word processing skills
  • Comfortable with remote application such as Zoom for team or other meetings
